"Saint Francis has taken a multidisciplinary approach to the management of congestive heart failure (CHF) patients. We implemented the first-of-its-kind heart failure disease management program. We have a team that meets daily to review important aspects of each patient's care. We have documented improved core measures, with patients more likely to receive proper discharge education, life-saving medications, appropriate testing, and smoking cessation counseling if cared for in our dedicated unit.
