At Masonicare Partners Home Health, we understand that most people would rather recuperate in their own home with the support of family and friends.

Saint Francis Hospital and Medical Center established a home health agency in 1976 to bring skilled home nursing and health-related services to residents in more than 20 towns in Hartford County. In 1996, the Suffield Visiting Nurse Association joined Masonicare Partners Home Health care to provide home health services in North Central Connecticut.

The Masonicare Partners Home Health staff includes experienced nurses with a wide range of expertise including such specialty areas as cardiac, psychiatric, oncology and maternal/child health nursing. Home care nurses and therapists working with you, your physician, family and other health care team members assess your needs and develop a personalized plan of care. This comprehensive approach to care will help you avoid unnecessary hospitalizations and recover more quickly.

Eligibility

To receive home health care you must be under the supervision of a licensed physician and meet reimbursement requirements of your health insurance plan. Otherwise, you may pay directly for services or negotiate a reduced fee. Essential therapeutic services will not be terminated because of inability to pay.

Masonicare Partners Home Health is a non-profit voluntary agency that is both Medicare and Medicaid certified. The agency participates in most health insurance and managed care plans.

Masonicare Partners Home Health is licensed by the State of Connecticut and accredited by the Community Health Accreditation Program .

Hours: Services are available 24 hours a day, 365 days a year. Office hours are Monday through Friday, 7:30 a.m. to 5 p.m.
