We are very excited for you to start your experience at Saint Francis Hospital and Medical Center. First we will have you complete our orientation and on-boarding. Note that in addition to our usual health clearance requirements, we have also implemented a mandatory flu vaccination policy, and any learners joining us during the flu season (December 1 - March 31) will need to obtain the flu vaccine and provide documentation of such along with their health clearance form.
Confirm with your preceptor that your educational institution has an active affiliation agreement* with Saint Francis Hospital and Medical Center.
*You cannot proceed further if your educational institution does not have an active affiliation agreement with Saint Francis Hospital and Medical Center.

If this is your first time at Saint Francis and you have not received health clearance from Occupational Health previously, please use the Exhibit D form for new learners. If you have done a rotation at Saint Francis previously and received health clearance from Occupational Health, please use the Exhibit D form for returning learners.
Have the appropriate form completed by your physician or school’s health center within four weeks before your start date. Send the completed form via fax to Occupational Health at (860) 714-8068

Download and read the New Learner Orientation Information Packet, Corporate Compliance Plan and Code of Conduct booklet, and Pocket Guide below. At the end of the Orientation Packet there are three exhibits (E, F, G). Print and bring these exhibits on your first day to review and sign with your preceptor, and ask any questions you may have about these materials at that time. Print the Pocket Guide and keep it on you for reference.

After viewing the presentation, click the link beneath the video to download the Sign-Off Sheet. Follow the instructions on the Sign-Off Sheet to complete and return it.
**Please note: Some areas request the documents from Step 4 and Step 5 (Exhibits E, F, and G and the Language Services Training Presentation Sign-Off Sheet) to be submitted in advance in addition to bringing them with you on your first day to review. Please be prepared to send these documents two to four weeks prior to your start date, if requested.
Download and print the badge form. You must complete this form with your preceptor and they must sign the form. Badging is done on the Saint Francis campus on Monday mornings, 7:30 – 11:30am and on the Mount Sinai Campus on Tuesday mornings from 7:30 – 11:30am.
Prior to arriving for your first day, confirm with your preceptor where you should park. Available parking is limited and you will be assigned temporary parking after you are issued a Saint Francis photo ID badge.
On your first day:
Bring your completed Exhibit D*, the badge form, Exhibits E, F, and G, and the Language Services Training Presentation Sign-Off Sheet.
- Review and sign E, F, and G with your preceptor. Give your preceptor copies of the Exhibits and the Language Services Training Presentation Sign-Off Sheet.
- Complete the badge form with your preceptor and obtain their signature
- * If during the flu season*: Take Exhibit D and the badge form to Occupational Health; obtain the flu tag and signature on the badge form
- Take the signed badge form to security to obtain a photo ID badge
Badging is done on the Saint Francis campus on Monday mornings, 7:30 – 11:30am and on the Mount Sinai Campus on Tuesday mornings from 7:30 – 11:30am.
Download and print parking form. Please complete the form in full and fax it to LAZ Parking as indicated on the form. Request that your parking assignment be emailed to your preceptor and your parking tag sent to your preceptor via interoffice mail. Please be sure to include your preceptor’s name and contact information on the form.
