New Learner OnBoarding Process

    We are very excited for you to start your experience at Trinity Health – New England, Inc. (TH-NE). First we will have you complete our orientation and onboarding. Note that in addition to our usual health clearance requirements, we have a mandatory flu vaccination policy, and any learners joining us during the flu season will need to obtain the flu vaccine and provide documentation of such along with their health clearance form.

    Step 1:

    Confirm with your preceptor/coordinator that your educational institution has an active affiliation agreement* with TH-NE.

    *You cannot proceed further if your educational institution does not have an active affiliation agreement with TH-NE. 

    Step 2:

    Part 1**: Download and print the Clinical Experience Participation Agreement. Complete, sign, and submit this form to your TH-NE preceptor/coordinator. This form must be signed and submitted in addition to you completing the electronic form below. 

    Part 2: Complete and submit the following online form:

    If this is your first time at TH-NE and you have not received health clearance from Occupational Health previously, please use the health clearance form for new learners. If you have done a rotation at TH-NE previously and received health clearance from Occupational Health, please use the health clearance form for returning learners.

    Have the appropriate form completed by your physician or school’s health center at least four weeks prior to your start date. Send the completed form via fax to Occupational Health at (860) 714-8068.

    Please note:Hepatitis B Waiver Form is required if learner has not received the vaccination series. Print, complete, and fax the Waiver Form along with your health clearance form to Occupational Health.

    Retain a copy of your health clearance form. If your rotation is during the flu season you will need to take your health clearance form with verification of your flu vaccine to one of the flu clinics where you will be given a flu validation badge. Read a message about our mandatory flu vaccination program and view the 2016 Flu Shot Clinic Schedule. 

    Step 4**:

    Download and read the New Learner Orientation Information Packet, Corporate Compliance Plan and Code of Conduct booklet, and Pocket Guide below. At the end of the Orientation Packet there are three forms – the Learner Mandatory Orientation Sign-Off Sheet, Confidentiality and Non-Disclosure Statement, and Corporate Compliance Plan and Code of Conduct Acknowledgement Statement. Print and bring these three forms on your first day to review and sign with your preceptor/coordinator, and ask any questions you may have about these materials at that time. Print the Pocket Guide and keep it on you for reference.

    Step 5**:

    Language Services Training Presentation

    Download and read the Language Services Training Presentation PowerPoint. After reading through the presentation, click the link below to download the Sign-Off Sheet.

    Follow the instructions on the Sign-Off Sheet to complete and return it.

    **Please note: Some areas request the documents from Step 2, Step 4, and Step 5 to be submitted in advance in addition to bringing them with you on your first day to review. Please be prepared to send these documents two to four weeks prior to your start date, if requested. 

    Step 6:

    Badge Form 

    Download and print the badge form. You must complete this form with your preceptor and they must sign the form. Badging is done on the Saint Francis campus on Monday mornings from 7:30 – 11:30am and on the Mount Sinai Campus on Tuesday mornings from 7:30 – 11:30am. 

    Step 7:

    Prior to arriving for your first day, confirm with your preceptor/coordinator where you should park. Available parking is limited and you will be assigned temporary parking after you are issued a photo ID badge.

    On your first day:

    Bring your Clinical Experience Participation Agreement, the Orientation Packet forms, the Language Services Training Presentation Sign-Off Sheet, the badge form, and your completed health clearance form*.

    1. Review and sign the Orientation Packet forms with your preceptor/coordinator. Give your preceptor/coordinator copies of those forms, your Clinical Experience Participation Agreement, and the Language Services Training Presentation Sign-Off Sheet.
    2. Complete the badge form with your preceptor/coordinator and obtain their signature. Bring the completed form to the Badge office during badging hours in order to obtain a photo ID badge.
    3. *If during the flu season*: Take your completed health clearance form to one of the flu shot clinics (see schedule above) to obtain a validation badge.

    Badging is done on the Saint Francis campus on Monday mornings from 7:30 – 11:30am and on the Mount Sinai campus on Tuesday mornings from 7:30 – 11:30am. 

    Step 8:

     Download and print parking form. Please complete the form in full and fax it to LAZ Parking as indicated on the form. Request that your parking assignment be emailed to your preceptor/coordinator and your parking tag sent to your preceptor/coordinator via interoffice mail. Please be sure to include your preceptor’s or coordinator’s name and contact information on the form.
