Individual Diabetes Assessment

Our program begins when one of our nurses or diabetes educators meets with you one-on-one to discuss your diabetes education needs, health goals, and current diet and exercise. If you need a glucose monitoring kit, we will provide one and teach you how to use it. You are invited to bring a support person, such as a spouse, family member, or friend, to this counseling session, as well as to subsequent group classes. 

Group Diabetes Education Class

Patients who have completed the individual assessment are ready to begin our two comprehensive, small group (from 8 to 10 participants) diabetes education program. Following is a sample schedule:

TIME  TUESDAY 
9:00-9:30 a.m. Diabetes overview
9:30-10:30 a.m. Nutrition Basics: What foods have carbohydrates?
10:30-10:45 a.m. Break
10:45-11:15 a.m. Oral diabetes medications
11:15-12:00 p.m. Exercise: What are the recommendations and how to get started
12:00-1:00 p.m. Blood sugar check and lunch
1:00-1:45 p.m. Preventing foot problems
1:45-2:15 p.m. Insulin and injectable diabetes medications
2:15-2:30 p.m. Blood sugar check

TIME  WEDNESDAY 
9:00-10:30 a.m. Nutrition: an individualized meal plan will be provided
10:30-10:40 a.m. Break
10:40-11:00 a.m. Preventing, recognizing, and treating hypoglycemia
11:00-11:30 a.m. Controlling diabetes when other conditions (even a cold) make you sick
11:30-12:00 p.m. Lifestyle change goals, resources to support you
12:00-1:00 p.m. Blood sugar check and lunch
1:00-2:15 p.m. Preventing diabetes complications and follow-up plan*
2:15-2:30 p.m. Blood sugar check

*Follow-up appointments are offered after eight weeks to review your progress in making healthy lifestyle changes.

Remember to bring your parking ticket. Parking tickets are validated at the center, so it costs only $1 for a full day in the garage.
