Getting Started at the Dental Center
Your first appointment will be a comprehensive examination with one of our doctors. New patients often state that their first appointment is the most thorough dental exam they have ever received. Following the review of X-rays and intra-oral pictures, we will begin a dialogue concerning your treatment. We'll educate you about your dental health, and then tailor a plan to fit your dental, financial, and personal needs.
We participate with the following insurance companies:
- Delta Dental® of New Jersey
- Anthem®/Blue Cross® Blue Shield®
We are more than willing to submit claims to other insurance providers if you do not belong to one of the plans named above. Please contact your insurance provider to discuss eligibility and benefits. Saint Francis employees are eligible for payroll deduction as a method of payment. We also accept cash, check, Visa®, and MasterCard®. The assistance of our financial coordinator is available to you upon request.

The following forms are available in our dental offices. To save time you can use the links below to print out the forms, fill them out, and bring them with you to your first visit.
In order to provide you with BestCare for a Lifetime, we need you to provide us with specific information. Before your first visit, please print and complete the patient registration and medical history forms 1 and 2 from the list below, and bring them with you to the Dental Center. Our HIPAA form is also provided. Please print, read it carefully, and bring it with you as well. We'll ask you to sign it in our offices during your appointment.
Your dentist or a Dental Center staff member may ask you to consult and/or complete other forms from the list below, as appropriate.
