Yes, upon request you may receive a patient visit summary report. Your report includes your diagnosis, medication information, and instructions for follow-up care, if any.

Will my primary care physician (PCP) be notified of my visit?
Yes, with your consent and by your request we would be glad to notify your primary care physician about your visit and release your records to him or her. We will ask you to sign consent forms permitting us to release your records to your PCP. We will also encourage you to see your PCP following your visit to Saint Francis FastCare, and to keep your PCP informed and up-to-date on all of your treatments and medications.

What if my condition unexpectedly requires a physician’s attention?
Your Saint Francis FastCare APRN will refer you to physician, if needed.

Does Saint Francis FastCare provide emergency medicine or urgent care?
No. For life-threatening medical emergencies dial 911. For urgent care needs such as treatment of cuts or sprains, visit your nearest urgent care clinic.
